  • Transcript –  Your school counselor or school official must send an official copy of your high school transcript with all of your grades.
  • School Report – The School Report form, which can be accessed through the Common Application website, has to be completed and submitted by a school counselor or school official.

The Midyear Report contains information about your GPA and class rank, and shares whether there have been any course changes or other updates since the submission of your application. It is accompanied by a Midyear Transcript which includes your most recent grades. If you are not given midyear grades or evaluations, you should add a note explaining your circumstances under the “Additional Information” heading on the writing section of the Common Application.
  • Counselor Recommendation – keep in mind that the School Report and Counselor Recommendation are two separate items on the Common Application and should both be completed.

The Final School Report and Transcript should be submitted online through the Common Application by your school counselor or another school official. In the Final Transcript will be your final grades and a confirmation of your completion of your secondary education.
